Neither option is automatically better. The right chair depends on the user’s strength, confidence, seating requirements, usual environment and whether a family member, carer or support worker will regularly be pushing. It also depends on the practical details that are easy to overlook, including wheelchair weight, transport, door widths and the condition of local paths.
What is an attendant-propelled wheelchair?
An attendant-propelled wheelchair is designed to be pushed from behind by another person. It has small rear wheels rather than the large rear wheels and hand rims found on a self-propelled chair. The person pushing uses the handles, while the wheelchair user is positioned comfortably and securely in the seat.
These chairs are often called transit wheelchairs. Their smaller wheels can make them lighter and more compact, which is useful when the chair needs to fit in the boot of a car, be stored in a smaller home or be taken on appointments and outings. A self-propelled wheelchair usually provides greater independence and often handles uneven outdoor surfaces better because of its larger rear wheels. It can also be easier for an attendant to push over small obstacles than a very small-wheeled transit chair. The trade-off is that it is generally larger, can be heavier and requires enough room for the user’s hands to reach and push the rims.
An attendant-propelled wheelchair is usually more compact and may be easier for a carer to transport. It is a sensible option where the chair is mainly used for supported trips and the user will not be left needing to move independently. There is also a middle ground. Some people use an attendant chair for travel and appointments, then choose a self-propelled chair for regular use at home or in the community. Questions to ask before choosing a wheelchair

Next, consider physical capacity. Self propulsion requires more than arm strength. It involves grip, shoulder range, trunk stability, coordination, endurance and the ability to brake safely. Seating and safety should not be an afterthought

A basic chair may suit occasional transport, but someone spending hours in the chair may need a more considered seating setup.

Positioning belts, lateral supports, headrests and elevating leg rests may also be relevant, but they should be selected for the person’s needs rather than added by default.
Brakes deserve attention too. Wheel locks on a self-propelled wheelchair should be within reach and secure enough for transfers. Allow for ongoing use, not just the first purchase
Manual wheelchairs need maintenance to remain safe and easy to use. Tyres, tubes, castors, brakes, hand rims and footplate components wear over time. A chair that pulls to one side, has worn tyres or loose castors can become tiring to propel and harder for an attendant to control.
Before choosing, check that replacement parts and compatible accessories are available.
